13. By checking the boxes below, you acknowledge that you have read and will comply with the elements of the agreement stated. By checking this box you understand that non-compliance will result in management relieving you of your responsibilities and asking you to leave and not return to the festival venue this year.

13a.

The purpose of this agreement is to clearly state the expectations for our support providers as we all work together to create a positive festival experience for our guests, our paid staff, and our volunteers.

13b.

I agree to represent the Bluesapalooza and/or my own Nonprofit in a trustworthy manner, with a high level of integrity, to follow the company’s established standards and any other local, state, and federal mandates.

13c.

I agree to be present and perform any responsibilities that I have been assigned in order to ensure the success of the festival.

13d.

I will not report for any shift while impaired nor bring alcoholic beverages into the venue at any time to share or be consumed by myself or others.

13e.

I will not consume alcoholic beverages while working/volunteering while wearing a worker/ volunteer wristband and or a worker/volunteer t-shirt while I am representing the Bluesapalooza in any capacity.

13f.

I, (and, if applicable, my Parent) on behalf of myself and each of my successors, heirs and assigns, unconditionally and irrevocably waive liability, release, and promise never to sue, forever discharge and relinquish and all rights, claims, demands, suits, actions, losses, damages, costs and expenses, including attorney’s fees and costs (collectively “Claims”), and I may incur or have against the Company, its affiliates including but not limited to Bluesapa LLC, Doheny Blues Festival, Inc., Omega Events, Inc., Mammoth Brewing Company, Sierra Event Management, the Mammoth Lakes Chamber of Commerce, the CCBA, and their respective officers, event production companies, and landowners/lessors. I (and, if applicable, my Parent) authorize each of the Released Parties to call for medical care for me or to transport me to a medical facility at my expense if medical attention is needed. I also authorize any physician or other medical provider or facility to provide any emergency medical/surgical care. I acknowledge and agree that none of the Released Parties is under any legal obligation to render assistance to me. By working and/or volunteering at Mammoth Festival of Beers & Bluesapalooza, you agree to assume all risks associated with COVID-19 and other infectious and/or communicable diseases, viruses, bacteria, or illnesses, as well as agree to all other terms set forth at www.MammothBluesBrewsFest.com. I acknowledge and understand that this Release is an important legal document, and by checking this box I am waiving substantial legal rights I may otherwise have to recover damages for injuries and losses, and sign it voluntarily and without inducement of any nature and intend for it to be endorsed to the greatest extent allowed by law. I am 18 years of age or older, or I am less than 18 years of age and I have the Parental Consent executed by my parent or legal guardian.
